Menopause is the point at which menstrual periods have stopped permanently, diagnosed after 12 consecutive months without one. Perimenopause is the run-up, when cycles turn irregular and symptoms usually begin. Common symptoms include hot flashes, night sweats, vaginal dryness, painful intercourse, low libido, disrupted sleep, mood changes, brain fog, joint pain, fatigue, weight gain and bladder issues.
Systemic hormone therapy with estradiol, as a patch, gel or tablet, is generally used for hot flashes, night sweats, sleep and mood. Local vaginal estradiol is used for dryness, painful sex and recurrent UTIs. Micronized progesterone is required alongside systemic estrogen for anyone with a uterus. Non-hormonal options for hot flashes include paroxetine and desvenlafaxine. This program offers four of these routes.
Systemic HRT is generally offered to women under 60, or within 10 years of menopause, who have bothersome symptoms and no contraindications. Contraindications include a history of breast cancer, heart attack, stroke, blood clots or unexplained postmenopausal bleeding. Women aged 35 to 39, or 66 and older, are typically eligible for local vaginal therapy only.
Estrogen on its own thickens the uterine lining and raises the risk of endometrial cancer. Adding progesterone protects the lining and sharply reduces that risk. Women who have had a hysterectomy are not required to take progesterone alongside estradiol, though some choose to for symptom relief.

Vaginal estrogen often takes 2 to 6 weeks. Systemic estrogen and progesterone for hot flashes, sleep and mood usually take 4 to 8 weeks, sometimes sooner. Timelines vary and results are not guaranteed.
Paroxetine or desvenlafaxine, which some clinicians use for hot flashes when hormone therapy is not a fit. Both are prescription medications with their own trade-offs, and your clinician will walk you through them.
